It is an alternative route to the UK which is becoming more complex by the year.
I have been on it since 2005 and can't lay claim to knowing it. All I can say is that it is a point based system where you may even score yourself before you apply. It is as black and white as it is presented. If your score add up to the required 75% you will most likely be granted. But I must tell you, that is the lightest part of the whole programme. On getting to UK you are on your own. They are not responsible for getting you a job or accomodation and you have no recourse to public fund. However you are required to renew your visa after 2 years and part of the requirements then is for you to show evidence of economic activity and you are also required to have made up to ?£40000(gross) in the 2 years. You are required to score up to 75% again in their point system. If you scaled thru this you will be granted a further 3 years leave to remain after which you are entitled to a permanent residency(Indefinite leave to remain). It is only at this stage you become a bonafide member of the populace with full right to public fund.
Being a doctor (coincidentally I am too) you need to remember that for you to get a job here you will need to pass your plab 1&2. You will need to do a clinical attachement of at least a month (possibly could last 3 months) after which you may start searching for the not readily available job. I am sorry if I sound scary but I think I need to let you know all so that you can take an informed decision. If you really have got something good going for you in Nigeria, i would advise you to look very carefully before you leap, the HSMP is like a poisoned chalice. The programme was most beneficial to those who applied for it several years back, and its been fine-tuned many times over, you might find it easier to scale thru the initial stage and get your 2yr clearance from 9ja, but its really difficult for many to meet up the requirements necessary for its renewal in the UK.
I know quite a few who had to return to Lagos cos the UKBA shifted the goal posts, and made it impossible for them to renew their visa - though they later got redress via HSMP forum ltd at the courts, but the deed had been done, not many of those that went back to their country took up the offer of coming back to the UK a second time to continue where they had left off.
If you are really keen on relocating, you might want to have a look at the canadian federal skilled worker programme. Its a very good scheme, the most favorable i've come across so far, and the canadian govt is very migrant friendly. You get a three-year visa, after which you qualify to apply for citizenship. No renewal hurdles here, its criteria is less stringent, and detroit - US is just closeby to Windsor-Ontario.
Its been overhauled very recently with a shortlist of eligible occupations, which they aim to process to completion between six months to a year under the bill C-50. There are many agents exploiting people over this, but you need not go thru anyone, just go to the cic website and check it out.

| Re: Whats the HSMP about I totally agree with Lumidii, it is very clear that UK no longer needs people to come into their country and take skilled/professional jobs, like it used to be, hence the enforcement of stricter rules not just for HSMP but students visa and other categories as well. For me, the better options remains Canada, the land of vegetation. For me, Canadian federal skilled worker programme is better than the UK HSMP, a friend of mine that just got it had to leave UK with his family for good after spending serveral yrs here without job (an experienced, top UK University graduate of Engineering [Msc] with distinction for that matter) I have seen a lot of medical guys, that has passed through this program, and end up being frustrated, some of them end up taking different kinds of odd jobs ---- Don't get me wrong, that is not to say that there are not ppl that got medical job after coming here under HSMP, but you have to also realise that UK has a good number doctors that are yet to be absorbed, nurses don't have much problem, the demand for nurses is high not just in UK but in USA as well. For you to integrate your self and compete, it may take you years, considering the fact that you don't have what they call "UK experience" of course you have to take your plab 1 and 2 as previously mentioned and may consider taking some other medical courses
It is your choice to make, but my suggestion is this, if you ok in Nigeria, there is no need going to UK, everywhere is the same, as day goes by, they make the rules stricter, suggesting the fact that they don't need foreigners "except very few when extremely required"
By the way, somebody told me that they have increased the total number of yrs for somebody on HSMP from 5 to 7, I am yet to confirm this, but I bet you, as far as the recession continues, there will be more and more changes in immigration
